I need hospital transport
Please note it is not the practice’s responsibility to book you hospital transport.
Somerset patients please phone the Patient Transport Advice Centre on 01278 727444 to see if you are eligible for free transport and to book. Their leaflet can be downloaded here. Briefly, you can get free transport if you either have severe mobility problems or are on any of the following benefits: Income Support, Income Based Job Seekers Allowance, Pension Credit Guarantee or have a Tax Exemption Certificate. Proof of this benefit needs to be sent in to the Patient Transport Advice Centre and must be dated within the last 3 months (Pension Credit Guarantee 12 months).
If you are not receiving a benefit then phone ‘Transporting Somerset’ on 01823 357713 or 01823 357688 to find out the cheapest transport options available.
Devon patients can phone 01392 494453.
